ITEMS TO REVIEW WITH LOCAL COUNSEL
- Client identity, contact information, age verification, and guardian process where applicable.
- Artist, date, service, placement, and a clear description of the planned work.
- Health disclosures and contraindications relevant to the service and local rules.
- Acknowledgment of disclosed risks, aftercare instructions, and when to seek medical help.
- Separate, optional permissions for photography, portfolio use, and marketing.
- Client and staff signatures, timestamps, version control, and correction procedures.
- Who can access completed forms and how records are retained, exported, backed up, and deleted.
- A review against current health-department, licensing, privacy, and consumer requirements in your location.
